Step 2:

Submit the completed order form

This can be done any way you feel most comfortable with. Phone, fax, mail, or email, it's all the same to us.

Call: 1-800-596-4266 M-F 8am-5pm CST. Monthly subscribers supply credit card number (Visa, MasterCard) and expiration date.
